Holy Cross Hospital Mountain Point (HCM) Locum Interventional CardiologistSporadic 24 hour (STEMI) call coverageCallback Hours:Extremely rareSchedule/Availability Requirements:Coverage needed is 24 hour STEMI call coverage.Shifts are 7a-7aDec 1 (7a) - Dec 3 (7a)Dec 23 (7a) - 29 (7a)Details:24 hour STEMI call coverage 7a-7aSporadic dates need October above.Looking for a provider to start as soon as credentialing (2-3 weeks).Setting: Inpatient, Call, Emergency RoomLevel 4 TraumaFTE: 1Required Procedures/Job Duties: Left and right heart caths, coronary angioplasty/stents, balloon pump/impella, transesophael Echo, pacemakers.Call: STEMI General CardiologyEMR: EPICSupport Staff: Echo Tech, Lab Tech, RN, X-ray Tech, Nuclear TechEMR Required Training:Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day. Credentialing Information:Timeframe: 2- 3 weeksRequirements: Board Certified Interventional Cardiology- RequiredActive UT license -Required or IMLC (LOQ in hand at time of submission)Active BLS -Required for CredentialingActive DEA -Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)Must state # of shifts per month you can offer to the team- RequiredCandidate must be available for oct shifts detailed in job description to be considered for ongoing shifts- RequiredClean Malpractice/background -Highly preferred - case by case - Must include self inquiry NPDB reportOnCall:Yes, 24 hour call to include 4 gratis hours WeekendDescription:YES Certifications:BLS,Board Certified,DEA StateLicenses:Utah
